I spoke with my brother (the guy who programs TheGiant.Org) regarding the uploading issue you have, and he had the following to say:
the root/wiki/images permissions and EVERYTHING IN IT need to be set to 777. that is read/write/execute for all.
WHAT IT SHOULD LOOK LIKE
## If you want to use image uploads under safe mode,
## create the directories images/archive, images/thumb and
## images/temp, and make them all writable. Then uncomment

there may be an option to turn on to make it so that email response is required to register.
Really what he should do is re install on top of the old database, but first drop the 'user' table in the SQL database, and just wipe out all the users. Then recreate the ones he wants.
But first backup the database just incase.
Not sure if that helps though. Let me know and I can forward any question you have, or even give you his eMail address and you can ask him directly. He's been doing wiki programming for some time now and has pretty much worked all of his issues out already.

In regard to your question as to how I see when spammers spam... I use the RSS functionality of my web browser to view this page (feed://wiki.howardforums.com/motorola/index.php?title=Special:Recentchanges&limit=500&feed=atom) which shows me all edits.
